During the coronavirus (COVID-19) pandemic, Prisma Wellness LLC is offering a virtual care option via video (telemedicine) for medical and wellness services only.

INSURANCE: We are credentialed with Anthem BlueCross BlueShield, Regence, First Choice Health, LifeWise, Providence, Cigna and Aetna insurances. We do not accept UnitedHealthcare or government insurance programs.

OUT OF POCKET: Out of pocket are billed at the time of service as follow:

• First Visit (Initial Eval): $350
• Follow-up Genetic Testing Review: $300
• Follow-up: $175
• Follow-up Med Management: $95

NEW CLIENTS: Please choose a tentative time and date for your appointment and a member of our team will call you to confirm your personal details within 24-48 hours. We do not take copays at the time of your visit.

Per our policy, all new clients will be charged a one-time deposit of $50 to hold the scheduled date and time, which will be deducted from the cost of the initial evaluation. If you have insurance, it will be applied towards your copay/deductible once we receive the explanation of benefits (EOB) from your insurance. Any remaining balance will be applied towards future visits or will be refunded if your insurance paid 100% of the visit or if you choose to not continue care with us. This deposit will not be refunded in the event of a no-show for your initial appointment without proper notice.

New client cancellations or rescheduling need to take place 48 hours prior to the scheduled date by sending a message via your patient portal or by calling us at 206-785-1015 and either speaking to a representative or by leaving a voicemail.
