COVID-19: Come and visit our new office in Bellevue! The use of face masks inside the premises is required for all in-person appointments. We are constantly working on sanitizing our public spaces, and have taken extra precautions, such as providing you with hand sanitizers and air purifiers in the most transited areas. We appreciate if you do your part to keep us all safe!

If you do not feel comfortable coming in yet, or if you are feeling under the weather, we are also offering a virtual option (via our secure video platform) for medical and wellness services.

INSURANCE: We are credentialed with Anthem BlueCross BlueShield (including Premera Blue Cross and Regence BlueShield), First Choice Health, LifeWise, Heritage, Cigna and Aetna insurances. Although we will submit a claim to your insurance, it’s your responsibility to verify coverage with your insurance. Our tax ID is: 843688679. Unpaid claims will be ultimately your responsibility. If your mental health benefits are administered by UnitedHealthcare, Providence, Optum or government insurance programs, these will not be accepted. If you have questions about eligibility, please call the back of your insurance card.

OUT OF POCKET: Out of pocket visits are billed at the time of service as follow:

Initial Evaluation: $350 (MD), $298 (NP)
Genetics Review: $300 (MD), $255 (NP)
Extended Follow-up: $250 (MD), $213 (NP)
Regular Follow-up: $175 (MD), $149 (NP)
Med Management Follow-up: $95 (MD), $81 (NP)
Document Preparation: $75 base line and $75 every 15 minutes (MD), $65 base line and $65 every 15 minutes (NP)

NEW CLIENTS: Please choose a tentative time and date for your appointment and a member of our team will call you to confirm your personal details within 72 business hours.

We do not take copays at the time of your visit. Per our policy, for a new client’s appointment reservation, we will collect a reimbursable amount to cover for your copay/coinsurance/deductible. If you are paying out of pocket, this will be applied towards the cost of your initial visit. Your card will be kept on file and will be charged when collecting copays/deductibles after future visits, when we receive the Explanation of Benefits (EOB) from your insurance.

New client cancellations or rescheduling need to take place 48 hours prior to the scheduled date by sending a message via your patient portal or by calling us at 206-785-1015 and either speaking to a representative or by leaving a voicemail.

WE ARE DOG-FRIENDLY PRACTICE: If you schedule an in-person initial consultation, you may be welcomed by one of our therapy wirehaired dachshunds. If you are afraid of dogs, allergic to dogs or have other reasons to not want to engage with a therapy animal, please let our team know when scheduling your appointment. If you want to come in with your therapy dog or any other support animal, please give our team a heads up so we can plan accordingly.
